WHAT IS RODO?
RODO is an EU law regulating the principles of personal data protection that we have been applying since 25 May 2018. Since that day, our facility has complied with the rules determined in RODO.
Full name of RODO: Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free flow of data and the repeal of directive 95/46 / EC.
WHO IS AN ADMINISTRATOR OF DATA?

The data administrator is responsible for security of the transferred personal data and its processing in accordance with law.
FOR WHAT PURPOSE AND ON WHAT LEGAL GROUNDS ARE WE USING DATA?
We will use your personal data obtained during registration to perform medical activities, provide health services, keep medical records and comply with our legal obligations. These obligations are determined in the regulations on the activities of healthcare facilities, in particular the Patient Rights Act and the Patient’s Ombudsman (in accordance with Article 6 paragraph c and Article 9 paragraph 2 point h of RODO).
Providing all data included in NHF declaration is a statutory requirement and is necessary for health services providing. The refusal to provide data will make it impossible for these services to be rendered by our institution.
HOW LONG WILL WE USE DATA?
We will use data for the period of health services and for the period in which the regulations of the Act on Patients’ Rights and the Patient’s Ombudsman order us to keep medical records. Depending on the type of documentation, it will be up to 30 years.
WHAT RIGHTS DO YOU HAVE?
You can submit an application to our facility for:
• access to personal data (information about personal data being processed and a copy of data),
• correcting data when it is incorrect,
• removing or limiting the processing of personal data.
You have also the right to submit a complaint to the President of the Office for Personal Data Protection if you believe that the processing of your personal data violates the law.
WHERE DO WE TRANSFER YOUR DATA?
Subject to data security, your data may be transferred − in addition to persons authorized by the Administrator − to other entities, including:
• other healthcare entities to ensure continuity of treatment and availability of health services;
• providers of technical and organizational services enabling the provision of health services by our institution (in particular to ICT service providers, suppliers of medical equipment);
• other entities processing them on our behalf (eg. medical laboratories)
